Patient Aligned Care Team (PACT)
What Is PACT?
A Patient Aligned Care Team (PACT) is each Veteran working together with health care professionals to plan for whole-person care and life-long health and wellness.
Primary Care/PACT is a team of caregivers (doctors, nurses, pharmacists, dietitians, etc..) who work together with patients to provide comprehensive medical care.

- Please fill out, print and bring this to your Primary Care Team appointment: My HealtheVet Healthy Living Assessment
- This Office of Patient Centered Care Personal Health Inventory allows the Veterans to communicate general health issues to improve communication with their Primary Care Provider during the first visit
- Your team includes:
- YOU - Veterans are the most important member of the Team
- Registered Nurse
- Primary Care Provider
- Licensed Practical Nurse
Health and You as a Whole Person:
The road to better health rests within you. The first step is to know what you want from your health and why. Knowing your health goals may not be a simple task; yet it is an important step toward reaching your full potential. Living life fully and optimizing health and well-being goes beyond just not being sick; it means understanding what matters to you and looking at all aspects in life that contribute to a sense of well-being. This tool will help you explore all areas of your life so your health care team can help you plan, not just for your medical needs, but for your “life” needs.
Health and You as a Whole Person:
This model represents “Patient-Centered Care” and how you are at the center of your PACT Team. Your wellness includes physical, mental, emotional, social, spiritual, occupational, and environmental factors.
If you would like to improve any of these dimensions, or are thinking about changing behaviors to promote your wellness, talk to any of your providers in your PACT team about behavior change, or request a referral to the Health Behavior Coordinator (HBC).
